I am enrolling my child/ward in this institution and I solemnly swear that I will obey the rules and regulations of the School. I will be diligent in the payment of the tuition fees based on the scheduled basis as well as the increase in tuition fees as may be allowed by the Secretary of the Department of Education. I will personally notify the MIS Office in case my child/ward is dropping and fully understand that my child/ward may be barred from further enrollment for failure to comply with the school academic requirements. We also understand that this agreement is for the entire school year, thus, we are bound to pay the tuition for the entire school year so that my child/ward be cleared from all financial obligations with the school. Furthermore, I hereby certify that my child/ward has not been found guilty in any disciplinary case for breach of conduct or violation of the rules and regulations or conditions of admission, nor had my child/ward been served notice that he/she cannot be re-admitted for certain infractions or charges or because he/she is suffering from any contagious diseases or ailments detrimental to the child's or other student's health. I hereby affirm that my certifications are true and correct and should it be found at any time that any statement is false, I am aware that this is a ground for my child's suspension and dismissal from school.

I have read the conditions stated herein and I have full understanding of my responsibilities and liabilities with Mary Immaculate School (Parañaque), Inc.. I agree with all of the legal constraints and repercussions. Mary Immaculate School (Parañaque), Inc. may hold against me should I violate any of the above-mentioned conditions. I understand that I am in contract with Mary Immaculate School (Parañaque), Inc. and that I subject myself under its implementing rules and regulations, and terms and conditions.
